Revised Sales and Use Tax Exemption Forms
The Colorado Department of Revenue has published the simplified sales and use tax exemption forms in accordance with House Bill 22-1039.
Please note that the revised form DR 5002:
- Is designed for wholesale and exempt entity purchases, including manufacturers;
- Should not have copies for machinery and machine tools sent to the Department;
- Does not require you to collect new forms from existing customers; and
- Allows you to use a different form or format to collect the required information in your records.
You will need to re-check the validity of a sales tax license or state-issued exemption certificate after its expiration date, or if you have reason to doubt whether the previous verification is valid or applicable.
